Fees

The fee is $145 for a 45-50 minute session. Elizabeth Berg PLLC knows that some need other arrangements in order for therapy to occur and has a limited number of reduced fee slots available, please check with Elizabeth regarding her current openings.

Elizabeth Berg PLLC accepts cash, credit card, or Paypal. The client will be held responsible for any transaction fee.

Insurance

Elizabeth Berg PLLC operates out of network with insurance companies and does not file insurance claims. However, the necessary information will be provided to you should you decide to attain reimbursement or seek payment from your Health/Flex Savings account. In order to seek reimbursement from your insurance company you will need to have "out of network" benefits on your plan. Please check with your insurance company.

Cancellation Policy

Since the scheduling of an appointment involves the reservation of time specifically for you, a minimum of 24 hours notice is required for re-scheduling or canceling an appointment. Unless we reach a different agreement, the full fee will be charged for sessions missed without such notification. In the event of extremely bad weather such as ice or snow, it is advisable to call, making sure the office is open. Please call/text if you are running late as I wait 15 minutes for late clients prior to leaving.

good faith estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.
